What to expect at your appointment
New patients should arrive at least 15 minutes prior to your scheduled appointment time. At your initial visit, you will need to allow yourself enough time to check in, have your IDs scanned into the system, and give our personnel time to enter your information. You will also need to bring the following documentation with you:
- Completed Registration Form
- Medical History, including prescription, over-the-counter medications and supplements
- Referrals and Authorization, if required by your Health Insurance Plan
- Insurance Cards

All major insurance plans accepted, including: Aetna, Anthem, Anthem Sr. Advan, First Health/Coventry, Great West, Humana, Ohio Health Choice, Medicare, UHC, AARP
We do not accept Humana Medicare PPO, Smartnet, TriHealth HMO, TriHealthSelect, Medicaid Amerigroup, Buckeye, Care Source, Molina, or Unison.
Please note that Humana Medicare Advantage Plans has delisted us from their plan against our will. We are unhappy that so many of our patients will no longer have access to specialty medical care. Please write to protest this decision to Humana and copy your congressional representatives.
A note from Dr. Coldiron:
“In 2014, CMS proposed to cut the reimbursement for dermatology-related phototherapy services by 50% to 60%. Doctors wrote lots of letters, but when patients write letters they look at them differently, and we ended up with a 9% cut instead. Together, patients and physicians are a powerful voice.”
