What to expect at your appointment

New patients should arrive at least 15 minutes prior to your scheduled appointment time. At your initial visit, you will need to allow yourself enough time to check in, have your IDs scanned into the system, and give our personnel time to enter your information. You will also need to bring the following documentation with you:

  • Completed Registration Form
  • Medical History, including prescription, over-the-counter medications and supplements
  • Referrals and Authorization, if required by your Health Insurance Plan
  • Insurance Cards

Please complete the New Patient Registration Form and Patient History and Intake Form ahead of time. This will help speed up your registration process. You can also access our Patient Rights here.

Accepted Insurance Providers

All major insurance plans accepted, including: Aetna, Anthem, Anthem Sr. Advan, First Health/Coventry, Great West, Humana, Ohio Health Choice, Medicare, UHC, AARP

We do not accept Humana Medicare PPO, Smartnet, TriHealth HMO, TriHealthSelect, Medicaid Amerigroup, Buckeye, Care Source, Molina, or Unison.

Please note that Humana Medicare Advantage Plans are including us in their plans December 1st, 2021.
